Private Fees and Charges

Not all services at the practice are available under the NHS. Where there is a requests for non NHS items or services a private fee may be payable. This leaflet lists some of those fees which may be payable in advance 

MEDICAL REPORTS, CERTIFICATES & FORMS (Without an Examination) CHARGES from £15.00 to £50.00 

Examples: Private Sick Note, Employment Certificate/Letter, Private Prescription, Private Medical Insurance, Shotgun Certificate, Holiday Insurance, Driving Licence, Seat Belt Exemption, Passport Form - Police/Teacher/Solicitor/Other Professional can sign your passport form 


Examples: HGV, Taxi, PSV, Council Reports, Employment Medicals, Complex Examination & Reports (Incl. Sports Certificate & Fitness, sky diving etc)

ACCESS to MEDCIAL RECORDS CHARGES from £10.00 to £50.00 

Photocopies are charged at 0.35pence per sheet plus admin charges - Access to Records Fee (plus any photocopy fee’s as below) Photocopy of Computerised Records and Paper Records up to maximum charge 

TRAVEL CERTIFICATES & VACCINATIONS CHARGES from £15.00 to £150.00 (Yellow Fever £50.00) 

Fitness to Travel & Freedom from Infection Certificates, Private Prescription for Drugs (Solely for Travel), International Certificate of Vaccination & Replacement Certificates, Meningitis A+C, Yellow Fever Injection (Required 10 days before travel), Rabies a Course of 3 Injections (Payable at First Appointment). Hepatitis B for Travellers a Course of 3 Injections (Payable at 1st Injection) 

HEPATITIS B - FOR EMPLOYMENT PURPOSES – Employers are responsible for providing vaccination for their employees & must request this as a private arrangement with the practice (Unfortunately we are not allowed to vaccinate our own patients privately). Patients registered with this practice must request their employer to provide vaccination with an independent practice or their own occupational health department. Aug 2014
